无障碍和住宿办公室（OAA）旨在为有残疾记录的学生安排专门的学术支持服务或改变学习环境。

学生可以在太平洋大学的学术追求的任何时候要求额外的住宿。 按照以下步骤开始使用OAA。

第一步：通过入学表格向OAA注册

• 登录到 适应 使用您的太平洋大学登录凭据查看您的学生门户网站。
• 在左侧“住宿”下，选择“入学表格”填写表格并上传您拥有的任何相关文件。 即使你不能回答所有的问题，也请尽力填写表格。
• Submit or request your 残疾的文档 from your service provider (doctor, therapist or evaluator). 学术标准要求 合格的文档 接受住宿。
  • 我有我的文件: Upload it into your 适应 Intake Form.
  • 我没有文件，但有一个提供者谁可以分享信息: You can request documentation from your provider or fill out a 残疾文档设计表单 to collect information from you and your provider. The form is first signed by you and is then automatically forwarded to the name and email address of 您指定的服务提供者. Alternatively, you can print a pdf of the Disability Documentation form and provide it to your provider.
  • 我没有文件，我不确定我是否有一个提供者，或者没有一个可以分享信息的提供者： Submit the intake form without your documentation and reach out to us at oaa@pacificu.edu. 如果您的残疾目前没有记录在案，而您需要进行残疾评估，有关评估的资源和信息可在“我可以在哪里接受评估”页面。

第二步：开会

  • A student who disagrees with an accommodation decision made by the OAA office staff may appeal or ask for reconsideration of the decision by making a request in writing to the Director of the 无障碍和住宿办公室, Victoria Bartunek (victoria.bartunek@pacificu.edu). If the student’s concerns are not resolved as a result of the appeal or request for reconsideration, the student may file a complaint with the Section 504 Coordinator (504 coordinator@pacificu.edu).

第三步：沟通

• 在住宿信发出后，我们鼓励您与您的教授预约会面，讨论您的学术住宿，并制定学期的理解和计划。 住宿信只寄给有记录的导师。 如果您的讲师或客座讲师没有收到此通知，请与他们分享这封信并发送电子邮件给OAA。 Some accommodations, such as 出勤灵活性 or accommodations related to deadlines, require the student and instructor to complete an agreement on how the accommodation will work in that particular class.
